Livingston County Health Department offers Homebound COVID-19 Vaccination Program

PONTIAC – The Livingston County Health Department (LCHD) is working to reach homebound individuals who are unable to visit COVID-19 vaccination sites. While all LCHD vaccination sites are ADA accessible, this program is specifically intended for Livingston County residents who are unable to leave their residence for medical reasons.
Eligibility for the program includes:
– Residency in Livingston County
AND
– Leaving home is not an option because doing so requires considerable and taxing effort.
AND
– Be a person with a disability who requires in-home assistance.
OR
– Need adaptive and/or accessible transportation to leave home.
For eligible residents, a form is available on LCHD’s website to notify the health department of your interest: https://lchd.us/site/covid-19-vaccination/. Once completed, the LCHD will reach out to you directly to schedule an in-home vaccination appointment. Those with limited access to technology may reach out to the LCHD directly at 815-844-7174 ext. 210.
It is essential that community members remain vigilant in the fight against COVID-19 by wearing masks and maintaining social distance, staying home when ill, and choosing to be vaccinated. Checking the LCHD website is the best way to receive up-to-date information regarding vaccine distribution at the health department, as well as updates regarding COVID-19 in Livingston
